Behavioral Health Case Manager
The Adult Case Manager provides necessary support and coordination of services to extend the ability of both Severe & Persistently Mentally Ill (SPMI) and non-SPMI persons over the age of 18 to live in the community in the most independent manner possible.
Responsibilities Include:
- Provides direct Case Manager services with SPMI and non-SPMI clients, their families, and the community at large
- Coordinate and participate in development of individualized comprehensive client treatment plans.
- Assists clients with basic money management; monitors client’s medical and psychiatric status; assists clients with daily living skills.
- Assists clients in identifying and accessing community resources.
- Provide coordination and assistance in crisis intervention and stabilization.
- Assist client in gaining formal and informal community and personal networking.
- Advocates for emotionally disturbed adults in our six-county area.
- Ensures the timely completion of all documentation related to providing and billing for services rendered.
- Maintain relevant documentation and provide statistical reporting.
Abilities/Skills:
- Must possess good organizational and communication skills.
- Must demonstrate good judgment skills.
- Able to utilize basic problem-solving skills.
- Able to resolve crisis situations in the most clinically effective manner.
- Must possess a valid Ohio driver license with acceptable driving record.
TECHNICAL: Must possess a minimum of an Associates degree in social services or related field. Bachelors degree preferred; Psychology, Counseling, Child Development or Social Work preferred.
ANALYTICAL: Exercises judgment in discriminating among many alternatives. Solutions to problems require the application of established practices. Reference to supervision and precedents is available as needed.
CONTROL: Monitors a complex process where corrective action is immediate and critical. Provides, at times, complex explanations to others that require some prior thought and planning.
